or if you want to order items at the same time you are returning items.
Return Form:
Download, print, and include [this] form with any items you need to return to INM
Crystal. Please be sure to properly package returned items so they are not
damaged in transit to us. Do not use regular envelopes to ship products back as
the items will be damaged in shipping. We will not issue refunds for damaged
items that were returned to us in regular envelopes.

if your order is over $1,000 US. We do this to cut down on fraud in order that
we may continue to provide you with the best prices possible.
Insurance Form :
Download Print, and Fax [this] form to INM Crystal at (561) 734-2321 if you
need to submit a request for a lost or damaged package.
